Student Health Checkup 2021 Information

※Please refer to the attachment file for the reception time
<Place> Entrance hall (1F)
<Items>
Chest X-ray examination、height、weight, blood pressure、
（only new students：urinalysis）
Please enter the " Health Questionnaire" before the Student Health Checkup
① Connect to the campus network, open the following URL (Moodle), and enter your Kyushu Institute of Technology ID and password.

②Please fill in the " Health Questionnaire".
● Please wear masks, line up at intervals, and do not speak privately in the venue.
●If you have a fever or cold(on the morning of the medical examination or going back to the past
two weeks ),you cannot have a Health Checkup and
please contact the Health center by email.
