① Connect to the campus network, open the following URL (Moodle), and enter your Kyushu Institute of Technology ID and password.

②Please fill in the "International Student Health Questionnaire".

③Please tell us the date and time (February 18th or March 1st) you would like to have a medical examination in your email titled "medical examination 2021" and attach the "International Student Health Questionnaire" which you have filled in.

<Preventive measures for new coronavirus infection>
●The following people will not receive a medical examination
・ Measure body temperature on the morning of the medical examination, and if you have a fever of 37.5 ° C or higher,

・ Those who have had obvious illness such as fever and cold symptoms going back to the past two weeks even if they
are not ill at present.
・ There is an infected person in a family member living together or an acquaintance with whom he / she has come into
