令和4年2月21日（月）・22日（火）4月7日（木）在学生の健康診断を実施します。

〈受付時間〉（完全予約制）9時～16時30分

〈場　所〉鳳龍会館

〈対象者〉令和4年4月時点での学部2年・3年・4年生

　　　　　大学院M2年・D2年・D3年

※就職活動等で健康診断証明書が必要な方は、2月の日程で受診してください。

※令和4年4月から進学予定学生は、別途で新入生健診を案内しますので、今回の案内は対象外です。

【健診Web予約方法】

①学内ネットワークへつなぎ、（学外VPN,学内KIT-1A,KIT-1B）

以下のURL（Moodle）を開き、、九工大ＩＤ・パスワードを入力する

②「私を登録する」のボタンをクリック

③「健康調査票」→「エントリーを追加する」→回答を入力

④予約入力（Active bookings）から希望の日時をクリックし予約する。

〈新型コロナウイルス感染症予防対策にご協力ください〉

●以下の方は、健康診断の受診を控えてください。

・健診当日の朝に体温を測定し、37.5℃以上の発熱がある方、咳、全身のだるさ、のどの痛みなど風邪症状のある方

・現在は体調不良はなくとも、過去２週間に遡って発熱・風邪症状等の明らかな体調不良があった方

・同居している家族や接触のあった知人等に感染者がいて、過去２週間以内に保健所により濃厚接触者・健康観察者と特定されていた方

●会場内では、マスクをし、間隔をあけて整列し、私語をしないように、ご協力をお願いします。

※発熱、風邪症状のある方は、保健センターへメールでご連絡ください。別途ご案内します。

※マスクのない方は事前に保健センターへ取りに来てください。

※その他、気になる点や心配なことがありましたら、保健センターへご相談ください。

[How to make a web reservation for medical examination]

① Connect to the campus network, open the following URL (Moodle), and enter your Kyushu Institute of Technology ID and password.

② Please fill in the "International Student Health Questionnaire".

③ Please make a reservation for the date and time you would like to have a medical examination.

★What to bring and prepare★

　　Student ID card, clothes that are easy to put on and take off, we recommend you to wear a plain T-shirt inside.

<New coronavirus infection prevention measures>

● The following people cannot receive a medical examination

・ Measure your body temperature on the morning of the medical examination, and if you have a fever of 37.5 ° C or higher,

Cough, general malaise, sore throat, etc. Those who have cold symptoms

・ Even those who have had obvious illnesses such as fever and cold symptoms in the past two weeks
