２０２６年度【学生健康診断】のお知らせ

今年度は4月の春休みを含む3日間で「学生の健康診断」を実施します。
学校保健安全法等において毎年1度は健康診断を受けることが義務づけられています。必ず受診しましょう！

 

〈日　程〉　　　4月4日（土）・8日（水）・１６日（木）　3日間のみ！

〈受付時間〉　9：0０～16：45（WEB予約制※）

〈場　所〉　　　鳳龍会館　

〈対象者〉　　２０２６年4月に在籍するすべての学生　

〈持ってくるもの〉　学生証　

 

※【健診Web予約方法】★健康調査票の入力も忘れずに！

①学内ネットワークへつなぎ、（学外VPN,学内KIT-1A,KIT-1B）

以下のURL（Moodle）を開き、九工大ＩＤ・パスワードを入力する

②「私を登録する」のボタンをクリック

③「健康調査票」→回答を入力

④予約入力（Active bookings）から希望の日時をクリックし予約する。

★健康調査票の入力後に予約ができます

 

〈注意事項〉

・健診受診にあたり配慮の必要な方は事前に保健センターへご連絡ください。　　　　　　　　　　　

・発熱、咳が出る等、体調不良の方は受診を控えてください。

・その他、気になる事や心配なことがありましたら保健センターへご相談ください。

[How to make a web reservation for medical examination]

https://reserve-mediex2.el.kyutech.ac.jp/moodle/course/view.php?id=2

① Connect to the campus network, open the following URL (Moodle), and enter your Kyushu Institute of Technology ID and password.

② Please fill in the "International Student Health Questionnaire".

③ Please make a reservation for the date and time you would like to have a medical examination.

★What to bring and prepare★

　　Student ID card, clothes that are easy to put on and take off, we recommend you to wear a plain T-shirt inside.

★ Please also note :

●Students who wish to be considerate when undergoing a medical examination should consult before the medical examination period.

●Students who belong to a company are obliged to undergo a medical examination at the company, so they do not need to undergo this medical examination.

● If you are looking for a job, please have a medical examination.

● The result of the examination will be notified with the medical examination certificate. You can use your student ID card from the automatic issuing machine in the Faculty of Engineering office.

● Those who have not undergone a medical examination cannot issue a health examination certificate.
